If you are not happy with the behavioral health services you are getting, you can use the Behavioral Health Services Department’s problem-solving process to try to fix the issue.

How can I start the problem resolution process?
- Fill out the Grievance and Appeal Form and send it to:
Quality Assurance Program, P.O. Box 28504, San Jose, CA 95159 - You can also call the Quality Improvement Coordinator at 1 (800) 704-0900 for help with your complaint or to talk to an on-call staff member from your health care plan.
